Faculty Notification

Procedure:

When students wish faculty notified of their need for accommodation, they must fill out the DSS authorization form ("Blue Form") to identify those faculty, courses and sections. Students are encouraged to notify their faculty during the first week of a semester.

(1) DSS will generate a notation to each identified instructor, advising (a) that the student is a member of the protected class and (b) of the required accommodations. Notations will be prepared within three days of receiving a student's "Blue Form".

(2) Students may elect to have notices mailed to their faculty but are encouraged to pick them up for personal delivery.

(3) Students are required to make an appointment to meet with each notified instructor to discuss their needs. Students should discuss each item on the Faculty Notification letter in a private, confidential setting.

(4) Students should contact the DSS staff if faculty (a) require clarification of or assistance with providing the annotated accommodations or (b) deny accommodations that have been duly documented.

(5) When requested, the DSS staff will assist students with approaching faculty regarding their needs by (a) employing coaching/role playing strategies and/or (b) direct liaison with the student and instructor.

(6) Every effort will be made to provide accommodations in a timely manner.

(7) Accommodations will not be provided retroactively, nor will grades be altered, when students choose not to notify faculty of their special needs.

(8) Students are encouraged to follow up with their DSS staff provider periodically during the semester to review progress, address problems and develop coping mechanisms and self-advocacy skills.

N.B. Students of the Extended Campus may choose to involve their center director to assist with the logistics of the above procedure.
